What's Needed for PreK Registration

The items listed below are required in order for your child's registration for PreK to be complete.  When all documentation has been submitted, the completed registration file will be considered for admittance based on enrollment priority requirements:
 
  • Proof of Birth - Government-issued Birth Certificate, passport, immigration documentation, decree of adoption, or other court-issued records.
  • Immunization Record- The student’s Tennessee Department of Health Immunization Certificate (shot record signed by the child’s physician or county health department).
  • Proof of a recent health/physical examination: The health/physical examination must have been completed within the past year.
  • Proof of Residence: One of the following—gas, water, or electric bill. If the utility bill and/or lease agreement is in the name of someone other than the parent/guardian, that individual must provide the utility bill and/or lease agreement, along with a notarized letter confirming that the parent/guardian and children reside at the address
  • Proof of Income for ALL working members of the household
 
Other documents that will be requested are: 
  • Copy of Insurance Card 
  • Social Security Card
 
To Obtain a Certified Copy of a Birth Certificate for a Child Born in TN
Parents can request and fill out an application at Morgan County Health Department located at:

To Obtain a Social Security Card
